There was a recent rumor started by the folks over at The Know that said Dark Souls 3 was in development. They even included some supposedly leaked images from the game that they somehow obtained (see bottom). Given how their last big rumor was immediately debunked and denied by Microsoft, there was much doubt that this rumor was actually true.
Today, IGN obtained a separate piece of media for Dark Souls 3 that seems to promote the fact that it will be out in "Early 2016."
Again, the game hasn't actually been announced yet, but given how there are now a number of leaks out there for this game, it's hard not to put up something about it. If anything, I would expect to see a Dark Souls 3 announcement during Sony's E3 2015 press event.
