Alright try this, I bet this is all of your problems.
Control panel > Mouse > Pointer options
MAKE SURE enhance pointer precision is on.. If it isn't, it will cause this "grid effect" and go like 2-3 pixels at a time instead of 1.
I used to have the problem with a Microsoft 400DPI mouse even with that setting ticked on - it's the mouse. I've had an MX518 for some 2 months now without that setting ticked, but at 1000hz, no problems whatsoever.
I just turned that setting on, completely forgot to reset that, thanks again.
Edit: Now I remember why I turned it off - that's responsible for mouse acceleration - it lowers your sensitivity if you don't give it "force". Couldn't stand that for CS, but maybe it's changed.
simple answer if you find your just missin his head move ur person a bit to the left or right then the grid moves so that his head is on a cross point. one of the sniper school vids on youtube talks bout it
NO! you shouldnt have to if you have a good mouse.

yea mouse accel, nice option for using on the desktop, but crap in games, only a plonker would enable it.
