I've had a few, such as I'm perfectly fine then out of nowhere it goes

in green text with no one who did anything. Or I'll just be running along in the middle of whore and it random says I killed someone with the weapon I currently have out. Even weirder is when it says I've killed someone with something I haven't used all round, like a claymore when I've only been assault.

One time this bug happened to me in 1.12, I was able to punish this guy for a tk when he did no damage at all to me, I got him banned from the server

If you automatically punish someone without considering the factors involved then you're a jackass.

This morning I apparently killed someone in Mashtuur using the J-10
