I'm also having trouble getting this... in situations where there are sniper fights are great because you the players doesn't instantly die from armour, grenades, etc and normally the enemy snipers take time for the reload. Wake island seemed to be a good one for this, also it's one big bottleneck where you get plenty of firefights (sometimes separated by a bridge). If you have a smart squad that is very active and doesn't just keep shooting til dead you'll have a better chance of getting it.
You could be the best medic in the world and still not get this. It's all about how smart your other team members are and it must be the right circumstances. It seems players would normally prefer to be revived than healed - just to keep shooting until dead.
