I think the majority of the problems could be solved with better case evaluation and a new classification of death: accident.
- Two buggies colliding frontally,
- burning chopper falls on teammates,
- friendly dies from explosion of an enemy vehicle,
- heat seeking missile hits friendly air asset after enemy used flares,
- friedly running over a mine/claymore placed at least 10 seconds ago,
- friendly jeep running into immobile tank,
- chopper pilot getting killed and chopper crashes with gunner,
- friendly hit by vehicle within 20 meters of vehicles spawnpoint
would be typical examples of accidents.
Harder to detect accidents would be friendlies running over a thrown grenade, friendlies entering into line of fire.
I'm sure you could add to this accident list too...
