Very possible but a little lucky to say the least.

A long time ago, I ran out of ammo and pulled out mah Takao, killed the guy in front of me but missed with the last shot and did a headshot on a guy > 30 meters away that I didn't even see - everyone were screaming hacks but I was just lucky

I once headshot somebody on the run that I was meant to spot. Somehow I missed 'q' key and hit him in the head. Ironically, that earned me an infiltrator pin.
