Started with 4. One just left because of horrible lag that day before the 2nd round. Then another left about round 11 to go do whatever. I died trying to run to revive and got stuck in one of the small doorways.

Most of the time people open up everything so we end up having to run everywhere. Almost 100% of the time I'm the last one when we've got to make a run for it and I usually get stuck and killed on that small balcony between the kitchen and the electric room.

The most successful groups I've played with make their stands on the balconies with the electric defenses. One person needs to keep each of the wooden barriers intact, while the rest keep shooting teh undead.

If your group uses the infinite ammo cheat (don't laugh...even with this, most groups will still get overrun eventually), there is another spot downstairs where 1 person can hold out for a long time (perhaps indefinitely, I'm not sure)...this splits the zombie horde, taking some of the pressure off of the group up top.

The groups I've played with that lasted the longest, usually used this basic approach:
            - guys on the German weapons side of the map will be the door openers (so they should be allowed to get more kills to get more $$$)
            - they will open everything up to the Power Room, throw the switch, but will not open the 2nd door in the room to the other side
            - everyone at this point should now buy the available perk-a-cola's, especially Juggernaut (1 extra hit goes a long way) and Quick Revive. Double-Tap is great, but it does burn through ammo quickly.
            - the guys on the US weapons side may want to open 1 door to get access to the Thompson/Double-barrel shottie, but if they do, they should not open the stairs in that room. FYI - opening up to the Power room, but not through it, and not opening up the stairs on the US side will deny you access to the Sleight of Hand quick reload perk, however.
            - the group should stay together as much as possible...it's very easy to make a mistake in higher levels and get trapped. If someone goes down, quite often it all falls apart when team-mates have to travel too far to revive him. If everyone is together, there's a better chance that 1 player can do the revive while the other 2 cover him.

            ..

I haven't yet tried out the opposite technique...of the US side doing all of the opening up. This would deny players the Double-Tap perk, but would allow Sleight of Hand, which may be more useful.

If using the infinite ammo cheat, then I think Double-Tap is better, because you don't have to reload - you can just blaze away firing. But if playing regular, then Sleight of Hand might be a better choice. Again, I need to try this with more people one night and see how it goes.

Actually, its about 3 or 4 hits without it and with it I noticed it took 6 hits when I got stuck one time.

                That's incorrect.

At the very start of the game, 2 hits quickly (without any time to regen health) and you're down in level 1.

With Juggercola, it goes to a minimum of 3 (possibly 4) hits.

The thing that throws you off is that the zombies usually attack in 2-hit combos - left/right arm swipes. So if you're facing one zombie, he'll hit you twice quickly, then your health starts to regenerate while he "resets" to swipe at you again. This makes it look like you can take more hits than actual.

If you get backed into a corner with multiple zombies where their swipes at you are more continual, on the 3rd or 4th swipe in quick succession (it's hard to tell sometimes, so we'll say 4 just for discussion) you'll go down.

If you get into trouble, sometimes you can stay alive for a few more seconds by jumping...by being in the air, some of the swipes will miss you and give you those extra fractions of a second to regen some health.
