Check to make user your mic is picking you up in the in-game audio options tab. Hit test 'n yap. If you hear yourself coming back, you're all set. Most likely, you've been playing on servers that have their VoiP mis-configured. You might also check to make sure you've not got the voice volume turned all the way down.
