Essentially it occurs with weapons and the other things.

M249 gun running around, sounds like hes shooting constantly.
Healing effect constantly running.
and supply effect constantly.

the m249 I experienced. the other 2 is other people in the server.

Yeah, sound loops are pretty common in my experience. Mostly, the healing sound loop, but an occassional gunfire one as well.
