Top left corner, I have seen it happen to other people sometimes but never to me. And I know for a fact I was alive and still alive when that came on screen.
Can anyone tell me what this means/ what causes the [ARTILLERY] DrDaz?
It means that a CMDr on teh other team sent an arty, but resigned from cmdr or disconnected when it hit u. People did this all the time before patch 1.03, cause they would get actual kill points for doing it. But it's been fixed now, and when that happens, nobody gets any points.

Its a bug thats been in the game since day one, the person it says is never dead, and the commander has never sent arty, it doesnt count against your score or increase the other persons.
Sometimes its to do with going commander, but its so random its hard to be sure.
yea, I have seen this a few times. Usually it says
[Teamkills] raceacontari
or
[Artillery] raceacontari
just like it does in the pic you showed. It once did this 3 times within a 20 second period while i was quite clearly alive...its just a bug...along with many other bugs in the game :\
