I am not sure whether it is a bug or intended to be this way, but I have earned a total of three purple hearts. My first I earned in on the Ghost Town map and it showed up immediately. The second two I earned a few days apart on the Leviathan map in Special Forces. The second two never showed up. I also browsed the awards of some on the leaderboards and haven't seen more than one purple heart. So I am pretty sure you can "earn" multiple purple hearts, but only one will ever show up in BFHQ. If I am wrong, please name the player who has more showing up in his stats. For that matter, name the player who has more than one Armor/Helo/Aerial medal as well.

It is possible to earn multiple purple hearts. However, I've earned a second one that never showed up. I tried to figure out why and finally came up with the reason. If you've been awarded a purple heart and then your K/D ratio goes above 1:4 (i.e. 6 kills, 20 deaths) it will actualy be taken away from you. This is what happened to me and at least one other person I've played with.
