Anyone know if there is a downloadable php or html page that i can put into my site to show my servers status/map etc..?

We use server spy on our site. I believe there is a free version and a pay version.

if you know the I.P is static(doesn't change) you could, or could use a small script to ping one of the many ports that you need open on the server. This is what i did for BF:V , by pinging the admin port i was able to tell the server was online and show some graphic. So I wouldn't pay for any script if I were you there are many pinging scripts out there.

o....errrrr...my bad you want more then the server being online or not. There was a project on source forge(another words FREE).
