What conditions occur when someone is [Killed] and can't be revived?

headshot, blown to bits, run over and crushed to name a few

the worst is when they die in the water or in some odd position and you can't revive them even though it says you can. that sucks because you usually stay out there too long and end up getting it yourself

Are you sure? As i have been killed before shot in the head or something similar, and i was revived.. and yes the words "You have been KILLED" Flashed on my screen, Not "Critically Wounded"
