Well, it took me a while to isolate the problem but I still can't find the problem. It only happens when you have more than 2GB memory available to your system. Pbuster randomly kicks you out after level loading with "unmodified content" message
I have already tried everything including:
1. Completely different hardware setups
2. Different OS: vista, xp
3. patching, latest pbuster, pbctl, drivers etc, etc
If you know what maxmem means then here is the fun part. If you have 4GB ram installed but set maxmem=2048 it works fine. But as soon as you increase it or remove it then problem comes back. So pbuster has issues with anything more than 2048MB of RAM? Hilarious
I have submitted this to EA and Pbuster, will see what they answer
