Its up to EA to decide. The only way you can give yourself a chance is to stay high on the leaderboard and hope you get picked at the end of the month.

Top 50 on the leader board each month. It's points then time for rank which is why some of these guys are either Lt. Gen or even Colonel.

cant remember where it was but i saw something about it being awarded to the best performing player of that month, so im guessing the Lieutenant General that accumulates the most points?

only the top 50 LT. Generals are promoted to 4 star. there are only 50 generals.

They are only Colonel because they lack either the awards needed for promotion or the time played. When they have this they will get whichever General rank that applies to their score/time played.
