Nothing, most people just like to have First Sergeant as opposed to Master Sergeant because First Sergeant is a higher rank, but it really doesn't matter.

Instead of a 1337 in-game diamond rank icon, you get a meh regular SGT add-on in-game rank icon. No difference. :p

First Sgt. is not a higher rank than Master Sgt. they are both E8's, First Sgt is just an advanced rank.

Actually... THERE IS A DIFFERENCE.

When you want to command, the First Sargeant will trump the Master Sargeant.

First Sargeant IS a higher rank.
