Open the console with the ~ key. Type in...
/name "whateveryournamewas"
There's no player database, per se. Just name yourself, but if you lost your rank.. You lost your rank. You'll have to regain it. :|

Dindy,
If your HDD was affected you may have corrupted the player config file.
If you look in C:\Program Files\Activision\Call of Duty 4 Modernwarfare\Players\Profiles\
You should then see a folder with your player name and inside will be three files: 2 x config files (one for single and one for multiplayer) and an mpdata
file.
The mpdata is the file that has your multiplayer score/rank etc and one the config file with "mp" in the name is the mutliplayer config file.
If you see these files, but cannot load the profile from the game then I guess the config file could be corrupt.
You could try simply saving a copy of the mpdata file somewhere else (like desktop) and the deleting the whole folder with your profile name. Then restart COD4 and create a new profile and player name, setup controls as you like and exit.
Then save a further copy of the new mpdata file that is now in the new profile folder (in case what we are about to do doesn't work), but make sure you give it another name like mpdata.old
Now save a copy of the "original" mpdata file from your desktop into the new profile folder and then restart COD4 and see if your old score/rank are available in the new profile.
If it goes screwy, then I guess the old mpdata file could also be corrupt and youll have to delete that one and replace it with the newer one you renamed and then earn your score back.
